Entire communities of sugar-farm laborers in Central America are being crippled by a sometimes deadly kidney malady — and nobody knows why. But some think the herbicide glyphosate, sold by Monsanto under the name Roundup, may be connected to the epidemic.

NPR reports on the rash of illnesses:

The first reports of this disease date back at least 20 years. At first the clusters of men dying of kidney failure was dismissed as a fluke. Then it was written off as diabetes or some other underlying health problem that hadn’t been correctly diagnosed.

Despite years of research all over the world, scientists still can’t definitively pinpoint the cause.

“We don’t know. That’s the unfortunate part, and we do desperately need to find some answers,” says Reina Turcios-Ruiz, a medical epidemiologist with the U.S. Centers for Disease Control and Prevention’s office in Guatemala City.

This form of kidney failure, known as insuficiencia renal cronica in Spanish (or chronic kidney disease of unknown origin in English), is now found from southern Mexico to Panama, Turcios-Ruiz says. But it occurs only along the Pacific coast.

The disease is killing relatively young men, sometimes while they’re still in their early 20s. Researchers at Boston University have attributed about 20,000 deaths to this form of kidney failure over the past two decades in Central America.

Chronic kidney disease has also shown up in rice-farming communities of Sri Lanka, leading the country’s government to restrict the use of Roundup and similar herbicides earlier this year.

Shortly before the Sri Lankan restrictions were imposed, a study published in the International Journal of Environmental Research and Public Health hypothesized a link between glyphosate and the kidney disease in areas with hard ground water that contains certain metals. “Although glyphosate alone does not cause an epidemic of chronic kidney disease, it seems to have acquired the ability to destroy the renal tissues of thousands of farmers when it forms complexes with a localized geo environmental factor (hardness) and nephrotoxic metals,” the researchers concluded.

The NPR reporter interviewed a victim of the mystery disease who is convinced that agricultural chemicals are to blame. “It was the chemicals, the chemicals at the plantation,” sickened Nicaragua sugar worker Manuel Antonio Tejarino said. “I feel like I’m burning. My blood pressure goes down. I get dizzy. Someone has to help me walk. If I’m alone I’ll fall down.”

What if Everyone in the World Became a Vegetarian?

The meat industry is one of the top contributors to climate change, directly and indirectly producing about 14.5 percent of the world’s anthropogenic greenhouse gas emissions, and global meat consumption is on the rise. People generally like eating meat—when poor people start making more money, they almost invariably start buying more meat. As the population grows and eats more animal products, the consequences for climate change, pollution, and land use could be catastrophic.

Attempts to reduce meat consumption usually focus on baby steps—Meatless Monday and “vegan before 6,” passable fake chicken, and in vitro burgers. If the world is going to eat less meat, it’s going to have to be coaxed and cajoled into doing it, according to conventional wisdom.

But what if the convincing were the easy part? Suppose everyone in the world voluntarily stopped eating meat, en masse. I know it’s not actually going to happen. But the best-case scenario from a climate perspective would be if all 7 billion of us woke up one day and realized that PETA was right all along. If this collective change of spirit came to pass, like Peter Singer‘s dearest fantasy come true, what would the ramifications be?

At least one research team has run the numbers on what global veganism would mean for the planet. In 2009 researchers from the Netherlands Environmental Assessment Agency published their projections of the greenhouse gas consequences if humanity came to eat less meat, no meat, or no animal products at all. The researchers predicted that universal veganism would reduce agriculture-related carbon emissions by 17 percent, methane emissions by 24 percent, and nitrous oxide emissions by 21 percent by 2050. Universal vegetarianism would result in similarly impressive reductions in greenhouse gas emissions. What’s more, the Dutch researchers found that worldwide vegetarianism or veganism would achieve these gains at a much lower cost than a purely energy-focused intervention involving carbon taxes and renewable energy technology. The upshot: Universal eschewal of meat wouldn’t single-handedly stave off global warming, but it would go a long way toward mitigating climate change.

The Dutch researchers didn’t take into account what else might happen if everyone gave up meat. “In this scenario study we have ignored possible socio-economic implications such as the effect of health changes on GDP and population numbers,” wrote Elke Stehfest and her colleagues. “We have not analyzed the agro-economic consequences of the dietary changes and its implications; such consequences might not only involve transition costs, but also impacts on land prices. The costs that are associated with this transition might obviously offset some of the gains discussed here.”

Indeed. If the world actually did collectively go vegetarian or vegan over the course of a decade or two, it’s reasonable to think the economy would tank. According to “Livestock’s Long Shadow,” the influential 2006 U.N. report about meat’s devastating environmental effects, livestock production accounts for 1.4 percent of the world’s total GDP. The production and sale of animal products account for 1.3 billion people’s jobs, and 987 million of those people are poor. If demand for meat were to disappear overnight, those people’s livelihoods would disappear, and they would have to find new ways of making money. Now, some of them—like the industrial farmers who grow the corn that currently goes to feed animals on factory farms—would be in a position to adapt by shifting to in-demand plant-based food production. Others, namely the “huge number of people involved in livestock for lack of an alternative, particularly in Africa and Asia,” would probably be out of luck. (Things would be better for the global poor involved in the livestock trade if everyone continued to consume other animal products, such as eggs, milk, and wool, than if everyone decided to go vegan.) As the economy adjusted to the sudden lack of demand for meat products, we would expect to see widespread suffering and social unrest.

A second major ramification of global vegetarianism would be expanses of new land available. Currently, grazing land for ruminants—cows and their kin—accounts for a staggering 26 percent of the world’s ice-free land surface. The Dutch scientists predict that 2.7 billion hectares (about 10.4 million square miles) of that grazing land would be freed up by global vegetarianism, along with 100 million hectares (about 386,000 square miles) of land that’s currently used to grow crops for livestock. Not all of this land would be suitable for humans, but surely it stands to reason that this sudden influx of new territory would make land much cheaper on the whole.

A third major ramification of global vegetarianism would be that the risk of antibiotic-resistant infections would plummet. Currently, the routine use of antibiotics in animal farming to promote weight gain and prevent illness in unsanitary conditions is a major contributor to antibiotic resistance. Last year the Centers for Disease Control and Prevention announced that at least 2 million Americans fall ill from antibiotic-resistant pathogens every year and declared that “much of antibiotic use in animals is unnecessary and inappropriate and makes everyone less safe.” The overprescription of antibiotics for humans plays a big role in antibiotic resistance, but eradicating the factory farms from which many antibiotic-resistant bacteria emerge would make it more likely that we could continue to count on antibiotics to cure serious illnesses. So what would be the result, in an all-vegetarian world, of the combination of widespread unemployment and economic disruption, millions of square miles of available land, and a lowered risk of antibiotic-resistant gonorrhea? I can only conclude that people would band together to form communes in order to escape capitalism’s ruthlessness, squat on the former pasture land, and adopt a lifestyle of free love.

I kid. Mostly. It’s easy to get carried away when you’re speculating about unlikely scenarios—and sudden intercontinental vegetarianism is very much an unlikely scenario.

But if the result of a worldwide shift to a plant-based diet sounds like a right-winger’s worst nightmare, it’s worth pointing out that continuing to eat as much meat as we currently do promises to result in a left-winger’s worst nightmare: In a world of untrammeled global warming, where disastrous weather events are routine, global conflicts will increase, only the wealthy will thrive, and the poor will suffer.

Let’s try a middle path. We’re not all going to become vegetarians, but most of us can stop giving our money to factory farms—the biggest and worst offenders, from a pollution and public health perspective. We can eat less meat than we currently do, especially meat from methane-releasing ruminants (cattle, sheep, goats, etc.). Just because a sudden global conversion to vegetarianism would have jarring effects doesn’t mean we can’t gradually reduce our consumption of meat, giving the market time to adjust. We not only can; we must. After all, with the world’s population slated to grow to 9 billion by 2050, we’ll be needing to take some of the 25 percent of the world’s land area back from the cows.

This week, when Ugandan President Yoweri Museveni approved a harsh new bill making “aggravated homosexuality” a crime punishable by life in prison, he cited a recent report from the Ugandan Ministry of Health’s Committee on Homosexuality, which concluded that same-sex attraction is mostly a learned impulse. “Since nurture is the main cause of homosexuality, then society can do something about it to discourage the trends,” Museveni said. “That is why I have agreed to sign the bill.”

This pronouncement creates a quandary for the United States. American officials, including Secretary of State John Kerry, have vehemently condemned Museveni’s decision. Yet millions of US taxpayer dollars are flowing to the agency that the Ugandan leader used to justify the legislation, according to records from the National Institutes of Health and the Centers for Disease Control and Prevention.

Gay rights activist argue that the Committee on Homosexuality report was engineered to ensure the bill’s passage, and at least one committee member—a physician named Eugene Kinyanda—refused to sign his name to it because the process had “taken a very political” direction. “I will not be used to justify the passing of a bill which as a doctor I do not fully understand,” he wrote in an email to a fellow committee member, which was reprinted on the blog Patheos.

How the West Virginia Spill Exposes Our Lax Chemical Laws

The West Virginia chemical spill that left some 300,000 people without access to water has exposed a gaping hole in the country’s chemical regulatory system, according to environmental experts.

Much the state remains under a drinking-water advisory after the spill last week into the Elk River near a water treatment facility. As much as 7,500 gallons of the chemical 4-methylcyclohexane methanol, which is used in the washing of coal, leaked from a tank owned by a company called Freedom Industries.

A rush on bottled water ensued, leading to empty store shelves and emergency water delivery operations. According to news reports, 10 people were hospitalized following the leak, but none in serious condition.

The spill and ensuing drinking water shortage have drawn attention to a very lax system governing the use of chemicals, according to Richard Denison, a senior scientist at the Environmental Defense Fund who specializes in chemical regulation. “Here we have a situation where we suddenly have a spill of a chemical, and little or no information is available on that chemical,” says Denison.

The problem is not necessarily that 4-methylcyclohexane methanol, or MCHM, is highly toxic. Rather, Denison says, the problem is that not a great deal about its toxicity is known. Denison has managed to track down a description of one 1990 study, conducted by manufacturer Eastman Chemical, which identified a highly lethal dose, in rats, of 825 milligrams per kilogram of body weight. But how that applies to humans at much lower doses in water isn’t necessarily clear.

In response to the crisis, the Centers for Disease Control and Prevention and the Environmental Protection Agency have determined that a level of 1 part per million in water is safe. The drinking water advisory is now slowly being lifted on an area-by-area basis.

So why do we know so little? All of this traces back to the 1976 Toxic Substances Control Act, or TSCA, the law under which the Environmental Protection Agency regulates the production of chemicals. According to EPA spokeswoman Alisha Johnson, MCHM is one of a large group of chemicals that were already in use when the law was passed, and so were “grandfathered” under it. This situation “provided EPA with very limited ability to require testing on those existing chemicals to determine if they are safe,” she says.

There are more than 60,000 such grandfathered chemicals, according to Johnson. A leak involving any of them into water could trigger to a similar situation of uncertainty—meaning that this spill has served to underscore a major gap in how we regulate chemicals.

“What we have now is a situation where because our system, our policies, and regulations don’t require this information be developed, we’re left scrambling when something like this happens,” says Denison.

Maps: The Mysterious Link Between Antibiotics and Obesity

Lately, I’ve been fascinated by a study on antibiotic prescription rates across the United States that was recently published in the New England Journal of Medicine. The researchers found a surprisingly wide variation among the states, and the rates—expressed in terms of prescriptions per 1,000 people—seemed to follow a geographical pattern: The Southeast had the highest rates, while the West’s were lower. West Virginia had the most prescriptions, and Alaska had the fewest. The rest of the country fell somewhere in between. Here’s a map of the findings:

As I thought more about the map, I wondered whether the prescription rates followed any demographic patterns. Lauri Hicks, a lead author of the study and a medical epidemiologist at the Centers for Disease Control and Prevention, told me that her team had initially expected to find certain correlations—for example, higher prescription rates in states with large elderly populations. But that didn’t turn out to be the case. Take Florida, which has a sizeable elderly population, but only an average antibiotic prescription rate.

Yet Hicks’ team did find one very strong correlation: The states with higher rates of antibiotic use also tended to have higher obesity rates. Take a look at this map of obesity rates by state and see how it reflects the antibiotics map above:

When we mashed up the data behind these maps, we confirmed the strong correlation between obesity and antibiotic prescription rates (we got an r of 0.74, for the statistically inclined). We also found a correlation between the states’ median household incomes and antibiotic prescription rates: States with below-average median incomes tend to have higher antibiotic prescription rates. This makes sense, considering that high obesity rates correlate with low income levels. (You can see the data sets for antibiotic prescription rate, obesity, and median household income level here.)

Hicks and her team can’t yet explain the connection between obesity and high rates of antibiotic prescription. “There might be reasons that more obese people need antibiotics,” she says. “But it also could be that antibiotic use is leading to obesity.”

Indeed, a growing body of evidence suggests that antibiotics might be linked to weight gain. A 2012 New York University study found that antibiotic use in the first six months of life was linked with obesity later on. Another 2012 NYU study found that mice given antibiotics gained more weight than their drug-free counterparts. As my colleague Tom Philpott has noted repeatedly, livestock operations routinely dose animals with low levels of antibiotics to promote growth.

No one knows exactly how antibiotics help animals (and possibly humans) pack on the pounds, but there are some theories. One is that antibiotics change the composition of the microbiome, the community of microorganisms in your body that scientists are just beginning to understand. Hicks says that more research is needed on the potential connection between antibiotics and obesity. But there are other reasons for doctors to change the way they prescribe antibiotics. As I noted a few weeks back, a recent study in JAMA Internal Medicine found that doctors commonly prescribe antibiotics for symptoms such as sore throat and bronchitis—which don’t usually require the drugs. Considering that bacteria are already evolving to withstand many antibiotics, it’s probably time to figure out how to use them more prudently.

The coal industry has a decades-old friendship with Johns Hopkins University, but now that cozy relationship is being torn apart by the scrutiny of investigative journalists.

When employees filed for black-lung-related benefits, coal companies paid the Baltimore-based university handsome sums to screen the claimants for the disease. After reviewing chest X-rays, the university’s scientists almost always concluded that the scans did not show black lung — a conclusion which often overwhelmed any other medical opinion in the case.

(Black lung disease, or coal workers’ pneumoconiosis, kills an estimated 1,500 former coal miners every year. It is a painful and preventable ailment contracted by inhaling coal dust.)

The racket was exposed by the ABC, working in partnership with the Center for Public Integrity:

For 40 years, these doctors have been perhaps the most sought-after and prolific readers of chest films on behalf of coal companies seeking to defeat miners’ claims. Their fees flow directly to the university, which supports their work, an investigation by the Center for Public Integrity and ABC News has found. According to the university, none of the money goes directly to the doctors.

Their reports — seemingly ubiquitous and almost unwaveringly negative for black lung — have appeared in the cases of thousands of miners, and the doctors’ credentials, combined with the prestigious Johns Hopkins imprimatur, carry great weight. Their opinions often negate or outweigh whatever positive interpretations a miner can produce.

For the credibility that comes with these readings, which the doctors perform as part of their official duties at Johns Hopkins, coal companies are willing to pay a premium. For an X-ray reading, the university charges up to 10 times the rate miners typically pay their physicians. …

In the more than 1,500 cases decided since 2000 in which [senior university scientist Paul] Wheeler read at least one X-ray, he never once found the severe form of the disease, complicated coal workers’ pneumoconiosis. Other doctors looking at the same X-rays found this advanced stage of the disease in 390 of these cases.

After the results of the investigation were broadcast late last week, the university announced on its website that it was suspending the screening program:

Following the news report we are initiating a review of the pneumoconiosis B-reader service. Until the review is completed, we are suspending the program.

United Mine Workers called on the federal government to take action following the revelations.

“Whatever penalties or punitive actions that can be taken with respect to Dr. Wheeler should be,” union spokesman Phil Smith said. “But whatever they are, they will pale in comparison to the pain and suffering he has caused thousands of afflicted miners. There is no penalty which will make up for that.”
